Privacy Statement

BALANCE for Blind Adults (BALANCE) is committed to respecting the privacy and confidentiality of our clients by ensuring policy, processes and procedures are established and adhered to in compliance with guidelines set out by PIHIPA.  The collection, use and disclosure of personal health information are within these guidelines, regulations and laws, with the respect of BALANCE clients the top priority.

Personal Health Information is disclosed by BALANCE in certain circumstances, either through explicit consent by clients, or for purposes that are permitted or required by law. These circumstances may include internal research to improve services, receiving payment from certain government agencies or a client’s private insurer and the release of information to a health care provider as per the request of a client.

Any staff member, client, volunteer or member of the public may contact the Privacy Officer, Deborah Gold, with any concerns related to BALANCE’s use of personal health information or any of its privacy practices.

The organization will work with you to resolve the issue. If you are not satisfied with the response, you have the right to file a complaint with Information and Privacy Commissioner of Ontario.
